Usually because the flush valve seal or inlet valve inside the cistern is worn, letting water flow into the bowl constantly. It's normally a quick parts replacement.
If the pan and cistern are in good condition, replacing the internal parts is usually the cheapest fix. If it's cracked, leaking at the base repeatedly, or parts are no longer available, a new toilet is often better value.
